Consent to Treat

I hereby give my consent as a legal adult or as a parent and/or legal guardian to providers at Steel & Grit Therapy LLC to provide medical treatment to the patient and/or to me through a virtual platform. I understand that this treatment will be provided by a licensed healthcare provider, and that all information shared during my online appointment will be kept confidential.

I understand that online treatment may have limitations and that an in-person examination may be necessary in some cases. I also understand that online treatment is not a substitute for emergency care and that I should seek emergency medical attention if I am experiencing a medical emergency.
